A Special Needs Plan (SNP) is a Medicare Advantage plan tailored to a specific group. A D-SNP serves people who have both Medicare and Medicaid (dual-eligible) and often includes very low costs and extra benefits. A C-SNP serves people with certain chronic conditions like diabetes or heart disease.
