Special Needs Plans (SNPs)
A Special Needs Plan (SNP) is a Medicare Advantage plan tailored to a specific group. A D-SNP serves people who have both Medicare and Medicaid (dual-eligible) and often includes very low costs and extra benefits. A C-SNP serves people with certain chronic conditions like diabetes or heart disease.
Dual-eligible (D-SNP)
If you qualify for both Medicare and Medicaid, a D-SNP can offer $0 premiums, low or no copays, extra allowances for groceries and OTC, transportation, and care coordination. We'll check your eligibility.
